B-Balanced wants to comply with the new guidelines of the General Data Protection Regulation (GDPR). In this e-mail we want to let you know how we handle your data and what we use them for. The most important change is that we must be able to demonstrate that we have permission to collect your data. Below we would like to give an overview of the data we collect and how it is stored and used.

 Which data do we collect:
- Your first and last name
- Your address details (possibly postal address when needed), telephone number and e-mail address
- Your intake form and / or anamnesis with relevant (health) data
- Notes of your treatment (s)

Why do we collect this data:
- Your first and last name is used for contact about appointments and any cancellation thereof.
- Your address details (possibly postal address when needed), telephone number and e-mail address are used for making appointments, sending notes, reminders, invitations and information about services and activities of our practice.
- Your intake form and / or anamnesis with relevant (health) data is used to make a treatment plan and to monitor the progress of the treatment (s).
- Notes of your treatment (s) are used to monitor the progress of the treatment (s).

How do we collect this data:
We collect your data by you filling in registration and contact forms on the website.
 
Where are your data stored and who are they shared with:
- Informer: accounting system
- Mijn Domein: Website maintenance
- MailChimp: Newsletter system
- Tax authorities

Your data will never be shared with third parties for commercial purposes. With everyone with whom your data is shared, a processing agreement has been concluded so that your privacy is guaranteed.
We need your permission to keep your details in our records and to send you a newsletter.
If you wish us to delete your data from our records, please send an e-mail to info@b-balanced.eu. We will then immediately delete all your data. This also means that you will no longer be kept informed of actions at B-Balanced.

For the tax authorities we are obliged to keep your data for 7 years in our financial administration. In that system, your data will be stored for 7 years, after which they will be deleted automatically.
